New in box, as pictured. I bought the red receptacle to indicate a solar backup inverter outlet, powered only when grid power fails. I don't expect to ever need the surge protection, just the LED power indicator on a hospital grade receptacle. Note that this does require a special PlugTail connector, and cannot connect to bare wires.
