These screen protectors were very hard to find but I found them here and they were correct. Unfortunately the shipping time was ridiculous. I didn't realize till later that the seller was in Germany. I won't be doing any outside of the United States shopping anymore it took way too long with customs and all.

The glass was packed in a polyethylene bag and a single-layer cardboard box. The box was the only rigid structure of the parcel. Obviously, it has not survived shipping - upper made-of-glass layer has shuttered in several zones.
I ordered glass screen-protectors from different suppliers previously, those glasses were packed inside rigid styrofoam cases.
